Buprenorphine-naloxone (Suboxone®) is effective for detoxification from heroin and prescription opioids. Detoxification is a process of medical supervised administration of buprenorphine to ease the discomfort of withdrawal and interrupt craving and drug use. Due to its unique pharmacology, buprenorphine is highly effective and safe in suppressing opioid withdrawal symptoms. Buprenorphine has been used in three ways for withdrawal from opioids: The long period withdrawal (>30 days), usually on an outpatient basis; moderate period withdrawal (>3 days but <30 days), again on an outpatient basis; and short period withdrawal (<3 days), which often is conducted on an inpatient basis. The available evidence shows that long-period buprenorphine detoxification is more effective than moderate- or short-period detoxification but that all forms of detoxification are less effective compared with ongoing maintenance. Avance Psychiatry offers outpatient medical detoxification using buprenorphine-naloxone (Suboxone®) or buprenorphine (Subutex®) or clonidine or lofexidine (Lucemyra®). We have developed standardized short term, medium term, and long term detoxification protocols that include buprenorphine and adjuvant medications to treat opioid withdrawal. It should be emphasized that detoxification is not completion of treatment and individuals should engage in outpatient individual or group counseling and monitoring to facilitate effective transition into ongoing recovery.
